In the United States of America , the medical licensing authorities of the various states grant a license to practice medicine and surgery. Each medical board sets its own rules and regulations and requires a passing score on an examination that demonstrates qualification for being granted a license. Results of the USMLE are then reported to these agencies for use in issuing the initial license to practice medicine. The USMLE provides the state boards with a integrated evaluation system for doctors for initial medical licensure. The USMLE examinations are sponsored by the Federation of State Medical Boards (FSMB), and the National Board of Medical Examiners (NBME).

Each of the three Steps of the USMLE is important; no Step can stand alone in the evaluation of qualifying for medical licensure.
